INFORMATION WE COLLECT

Information you provide. We may receive information that you choose to submit through the website, such as your name, email address, telephone number, account information, and the contents of a message or request.

Technical information. Wix and service providers used by this website may automatically collect information such as your IP address, browser and device type, operating system, referring pages, pages viewed, approximate location derived from your IP address, and dates and times of visits.

Cookies and similar technologies. The website may use cookies, pixels, local storage, and similar technologies. Some are necessary for security and basic site operation. Others may support preferences, functionality, analytics, or marketing. Nonessential cookies are used according to the choices available through our Usercentrics consent banner.

HOW WE USE INFORMATION

We may use information to operate, secure, maintain, and improve the website; respond to inquiries; provide requested information; understand website traffic and performance; prevent fraud or misuse; comply with legal obligations; and protect the rights and safety of the practice, our patients, and others.

MEDICAL INFORMATION AND HIPAA

This website is not intended for medical emergencies, diagnosis, or treatment. Do not send protected health information, urgent clinical questions, prescription requests, or other sensitive medical information through ordinary email, social media, or unsecured website communications.
